Please review all statements carefully, as your submission and signature will serve as a legally binding agreement and will be used to determine your eligibility for academic credit.
Attendance Sheet
Please complete, sign, and submit your attendance form to support our impact reporting and to receive your academic credits. All submissions must be made on or before the 15th day of each month.
